Request a birth certificate

Request a copy of the certificate in the municipality where you were born. You can choose between a copy in Dutch or an international multiple-language extract.

A birth certificate is the written proof that you were born. The municipality where you were born keeps this certificate in the Civil Registry.

  • A copy of a certificate is an official copy of the record. The copy is only in Dutch.
  • An extract of a certificate is a summary of the record. An international extract in multiple languages includes explanations in 10 languages: Dutch, English, French, German, Spanish, Greek, Italian, Portuguese, Turkish, and Serbo-Croatian.
  1. Apply for the birth certificate online

    Go to Apply and answer the questions first.

  2. Fill in the form and pay

    The municipality will review your request after you have paid.

  3. You receive the copy or the extract by post

    This is usually within 5 weeks.

Was the birth certificate drawn up more than 100 years ago? Then you will need to contact Begin link: The Hague City Archives, end link. . Do you need the birth certificate as an official document? Then you must request it from the municipality.

You can apply for the certificate for yourself or for 1 of the following persons:

  • your children or grandchildren
  • your parents or grandparents
  • your beneficiaries
  • your spouse
  • your registered partner
  • someone for whom you are the legal representative (you may make decisions for them)
  • someone who has authorised you (has given permission)

Apply for someone else

Are you requesting the birth certificate for someone else? Sometimes you need extra documents. Check when this applies and what you need.

For your application, you also need:

  • a copy of the ID of the person who gave you the authorisation (the grantor)
  • a completed authorisation form

  • a document that shows you are the legal representative. For example, a decision from the court.

For your application, you also need:

  • a copy of the certificate of inheritance. You can get this from the notary.

The municipality will send you the copy or multiple-language extract within 5 weeks. Delivery time depends on the postal service.

The following details are on your birth certificate:

  • first and last name(s)
  • the date and place of birth
  • the time of birth
  • sex
  • names of parent(s)

Do you need to have the document translated? You can have it translated by a sworn translator.

For documents which are needed within the European Union you can request a Begin link: multilingual standard form, end link.  along with the document from the Netherlands.

To use the document abroad, you need to have certain foreign certificates legalised (made official with a signature and stamp/’apostille’). More information can be found on the Begin external link: website of the Government of the Netherlands(External link), end external link..
