Consent for identity card for children younger than 12 years
To get an identity card (ID card) children need written permission from the people with parental responsibility. This is usually both parents. This applies to children younger than 12 years.
Who has to give permission?
All of the people with parental responsibility must give written permission.
- Do several people have parental responsibility? Then they all must give permission.
- If 1 of the parents does not have parental responsibility, he or she does not have to give permission.
- Is somebody else (also) the guardian of your child? Then this person must (also) give permission.
How do you give permission?
All of the people with parental responsibility must give written permission. How you do this depends on who will accompany the child to the appointment. This will also determine what you need to take with you.
You have 2 ways to give permission:
You and the other person(s) with parental responsibility will come together with the child to apply for the identity card. All of you take a valid identity document to the appointment. For example, a passport, driving licence or identity card (not a copy). You will fill in a consent form during the appointment.
1 parent/legal guardian will accompany the child to apply for the identity card. You take the following with you:
- A valid identity document for yourself. For example, a passport, driving licence or identity card (not a copy).
- A valid identity document for the other parent(s)/legal guardian(s). For example, a passport, driving licence or identity card. This can also be a copy. You have to submit the copy when you apply.
- The Begin external link: consent form(External link), end external link. for the other parent/legal guardian (not a copy). The form must be filled in completely. The signature on the consent form must match the signature on the identity document.
