Attestation de vita extract (proof of life)

The attestation de vita extract is proof of life. An agency which issues payments can ask you for proof that you are still alive.

      What does this extract contain?

      • your surname
      • your given name(s)
      • your date and place of birth
      • your home address

      Conditions

      • You are registered at an address in The Hague.
      • Are you not or no longer registered in the Personal Records Database (BRP) and is The Hague the last municipality in the Netherlands where you were registered? Visit the municipal counter in person with a valid form of identification. For a multi-language extract, first make an appointment at the municipality.

        You can also request an attestation de vita extract at the information counter at the Centrum and Escamp city district offices. You will get the extract right away. You can see the opening hours and the walk-in hours under the opening hours and locations of city district offices.

        Pension

        Are you about to retire or do you already receive a pension? Then you get this extract free of charge. In this case it is not possible to apply online. Request your free attestation vita in writing to the Department of Public Service. Send a signed letter, a copy of your valid form of identification and a copy of the written request from the pension fund.

        Fees

        € 14.60 in 2019. You pay this amount through iDEAL when you apply online. Are you picking up a multi-language attestation de vita at a city district office? You can pay in cash or use a bank card at the Centrum and Escamp city offices.

            • The extract will be sent to your home within 2 working days.
            • A multi-language attestation de vita extract will be issued to you on the spot.
              • The free attestation de vita extract will be sent to your home within 2 working days after the municipality receives your written request.
