Request a certificate for someone else

You can request a copy or extract of a certificate for someone else. This can only be done online or at a municipal counter at the city district offices.

Conditions

Would you like to request a copy or extract of a certificate for someone else? This is possible only if:

  • you are the (grand)child or (grand)parent (not a brother or sister)
  • you are a beneficiary
  • you are the spouse or registered partner
  • you are the legal representative
  • you need the certificate for a legal procedure
  • you need the certificate to get married or register an unmarried partnership

Do you need to get a certificate for somebody but you do not fall under one of the above categories? Then you need written consent (written authorisation). You can use the written authorisation form for the Registry Office (in Dutch). The form must be signed by the person for whom you are requesting the document. Include this document together with a copy of your identification as an attachment in your online application. Or bring it with you when you apply at the municipal counter.

    Make an appointment

    Bring the following to your appointment:

    • a valid form of identification
    • (a copy of) the identification for the person for whom you are requesting a copy or extract
    • written authorisation (if applicable)
