Want to become an organ donor?

Pick up a form at a municipal office.

The Municipality of The Hague aims to raise the number of organ donors. This is why the municipality is handing out donor registration forms to people who stop by one of the city’s eight sub-municipal offices. Register your consent to donate certain organs and/or tissue for transplantation.

It’s also possible to register your preferences in the Donorregister using your DigiD. You can also pick up a donor registration form at one of the sub-municipal offices

A life-and-death matter

Organ donation is a topical and important subject. There is currently a shortage of organs. Each year many patients die because they had to wait to long for a donor organ. It is a matter of life and death that you register as an organ donor.

Do you want to become a donor? Or not?

All people age 12 and older residing in the Netherlands can have their wishes regarding organ and tissue donation registered in the Donorregister. Registration provides clarity and certainty for everyone involved. Registering your wishes can only be done by using the organ donor form. This form is only available in Dutch. You can choose from four different options:

  • Option 1 Yes, I give permission
  • Option 2 No, I don't give permission
  • Option 3 My next of kin decide
  • Option 4 A specific person decides

Register your consent

In the event that somebody hasn’t registered his or her consent to donate organs, his surviving relatives will be asked for permission. This is a difficult decision which often needs to be taken at a highly emotional moment. If more people register as organ donors, more lives can be saved.
