The Special Care Indication Bureau of the GGD (Public Health Department) evaluates whether you entitled to sheltered housing, home care or admittance to residential care or a nursing home. The Indication Bureau also gives social-medical advice for parking permits, priority indications for housing, special care, work and education.
Available over the phone at (070) 305 38 00, Monday through Friday from 8.30 till 17.00 hrs. You can also phone this number for emergencies in connection with a request for home care.
Requests for residential care, nursing homes and sheltered housing must be submitted in writing. The registration forms can be requested at (070) 305 38 00.
Priority declarations for housing can be requested from the housing authorities. Parking permits from the Department of City Maintenance at (070) 353 68 83.
Requests for adaptations to your house are submitted to the Department of Social Affairs and Employment Opportunities at 0800 - 23 35 03. In such cases, the Indication Bureau will ask you to attend a medical examination. You should bring any documents provided by your doctor or specialist with you.
Indications for Special Health Care will be paid if you have AWBZ insurance (a public insurance for health risks). You will have to pay for certain permits yourself.
